02.21.03 - Boeing Debris Impact Assessment Charts
Three sets of charts, from Jan. 21, Jan. 23, and Jan. 24, were part of an analysis conducted during the STS-107 mission of the impact of external tank foam debris striking Columbia during ascent. The analysis was presented to the STS-107 Mission Management Team on Jan. 27 with the conclusion that the effects of the debris did not pose a safety of flight concern for Columbia. Mission managers concurred with that conclusion. These charts do not represent a comprehensive look at the analysis, which included extensive verbal communication, and took place over more than a week while Columbia was in orbit. All open work shown on these charts was completed and reported to Space Shuttle management.

Two email streams regarding the same "dire" scenario that could lead to loss of crew and orbiter have been located. Both email streams begin with Shawn Greenwell's email of 8/9/2000. One of the streams ends with Ed Sikora's reply on 8/11/2000 and the other ends with Fernando Caldeiro's reply on 8/14/2000. These emails address a specific scenario whereby there is a Back-up Flight Software (BFS) engaged abort on the General Purpose Computer (GPC) aboard the Orbiter, together with a loss of voice communications between the Launch Control Center (LCC) Control Room and the crew inside the vehicle at the Launch Pad. The email indicates that the launch team is comfortable with the present procedure configuration because of the low probability of a BFS engaged abort coupled with multiple redundancies of communication paths with the crew.

This document describes an "unexplained anomaly" involving the "A circuit" pyrotechnic devices for the MLP 3 hold down post (HDP), the ET vent arm system (ETVAS), and the ETVAS lanyard, which did not receive detonation energy as expected from the MLP 3 System A Pyrotechnic Initiation Controller during the launch of STS-112 on October 7, 2002. The report includes the analysis that was conducted to determine the possible causes for the failure of the A circuits, as well as the most probable cause and the corrective action instituted by NASA.
